Before your appointment
On your first visit, we’ll ask you to complete a registration form and sign an informed consent form.
Please bring anything that helps us understand your condition:
- Any scans, X-rays or radiology reports
- Medical reports and GP referrals
- Insurer or claim details
- Any other documents relevant to your treatment
Fees and payment options at our clinic
Private health insurance: bring your card and claim your rebate on the spot through our HICAPS terminal. Depending on your level of cover, a small gap may apply. As a Members’ Choice Preferred Provider with Medibank — and a preferred provider with most other major funds — members receive maximum rebates.
NDIS, WorkCover, TAC & DVA: we bill your provider directly, with no gap payment for these services.
Medicare (EPC plan): if your GP has referred you under an Enhanced Primary Care plan, bring your Medicare card. Bulk-billing physiotherapists are available on request, with no gap payment.
